Either way you would be looking for a machine shop to do the deed. I believe that bungie's suggestion of a nuclear warhead is actually a bit... conservative BTDT.

After doing some research, I think I will end up going with a Dremel carbide endmill, time, and patience.

I'm not normally a safety nazi, but carbide is very brittle. Using it in a non-rigid tool (dremel as opposed to a bridegport) it will break. Probably several of them will.
Carbide has an unnatural link to the unholy which guarantees a microfragment will take up residence in your cornea. As much fun as that sounds like, it really isn't. Medical bills are expensive. Pain is undesirable. Safety glasses are as inexpensive as they are dorky looking. Use them.
